Flu Immunization Clinic - October 28

Dear Parents/Guardians:

 

               Thrifty White of Granite Falls will be doing the Influenza Shot Clinic for the students at Lakeview.  The shots will be given on Thursday, October 28th, starting at 8:30.  I have enclosed a form that will need to be completed and returned to the health office by Friday, October 22nd, 2021 in order for a student to receive the influenza vaccine.  Each student needs to have his or her own form filled out with a copy of insurance card or insurance information written on the form. You may skip question #1 that asks if your student is sick today. The nurses will ask the students that question on the day of vaccine administration. The form is also available online on the Lakeview Public School website on the home page.  Students age 4 and older are eligible to receive the vaccine here at school.

 

Parents will not be allowed to attend or assist their child/children during the flu shot clinic. If your child is absent on this date, there will not be a makeup date scheduled. However, there are plenty of pharmacies and clinics that will offer the flu shot as well.

 

I encourage all students and parents to have their influenza vaccine annually unless your health care provider has other recommendations or has cautioned you against receiving any influenza vaccines.  Please note that ONLY SHOTS will be given at school this year.  The influenza nasal spray is available for this flu season just NOT at the school.

 

               Please continue to report to the school any students with a fever greater than 100.4 degrees who also have a cough or a sore throat.  This is required influenza-like illness reporting for the Minnesota Department of Health.  The Minnesota Department of Health also requires mandatory reporting of any contagious childhood illness diseases such as mumps, measles, rubella, chicken pox (in students or staff), shingles (only in children), pertussis (whooping cough), diphtheria, tetanus, polio, viral hepatitis, and COVID 19.

 

               I encourage parents to notify me, your child’s teacher, Mrs. Caron or Mrs. Devereaux if any other contagious conditions such as strep throat, impetigo, scarlet fever, mononucleosis, hand, foot and mouth disease or any other condition diagnosed by a physician occurs.
