Special Diet and Allergies

A special diet medical statement needs to be completed upon special diet diagnosis and updated whenever the student diagnosis or special diet changes.  Turn in special diet request to the school nurse or kitchen manager.   Upon written request from a parent, Lakeview School will provide lactose free milk.  If your child requires lactose free milk,  please contact the school at 507-423-5164 Ext. 1350.
